Report an Incident

Developers, deployers, users and affected persons can report incidents involving AI systems: events that caused harm, created a risk to public safety, or narrowly avoided doing so. Incident reports feed the assessment work of IRBAI and can lead to audits or enforcement where the facts warrant it.

For operators of certified systems, reporting of serious incidents is an obligation under the Statute and the conditions of the certificate, and should be made through this channel without delay.

If you are reporting suspected wrongdoing rather than an event, use the Report a Concern channel, which carries the protections of Chapter 18.


SUBMISSION

Before you submit

Report what is known now; completeness is not required. An initial report can be supplemented as facts develop, and early notification is more valuable than a complete one.

The application area and nature of the incident follow the risk classification framework of Annex I. Select the closest match; the assessment does not depend on your choice being exact.

For incidents involving immediate danger to life, alert your national emergency and supervisory authorities first. This channel does not replace emergency services.

The company or person responsible for the system, if known.
Areas follow Annex I Part 2 of the Statute.
Dimensions follow the assessment criteria of Annex I Part 1.
When the incident occurred or the period began. Use YYYY-MM-DD, approximate if necessary.
Leave blank if the incident is ongoing.
Street address, facility or site, or the online service concerned.
